Pain is one of the most complex mental and sensory human experiences and its linguistic representation is shaped not only by individuals’ characteristics, but also by linguistic structures and cultural context. This descriptive–analytical study investigates how pain is linguistically represented in the speech of Persian speaking patients within the framework of Halliday’s Systemic Functional Grammar while focusing on the ideational metafunction and the transitivity system. The data were collected through fieldwork and consisted of 25 face to face conversations purposely selected from among 120 interactions between the patients and ophthalmologists in Bushehr, Iran. The analysis examined the patients’ lexical grammatical choices with respect to types of experiential processes. The findings revealed that material processes (26 instances) and relational processes — attributive (19 instances) and possessive (10 instances) — were the most frequent resources used to represent eye pain. Overall, the predominance of material processes, followed by relational processes, in patients’ speech indicated that, through their association with actions, changes, and observable attributes, these processes enabled the concrete construal of an internal experience such as pain. Accordingly, in representing their eye pain, the patients relied on these processes to objectify and bodily construe their inner experiences, presenting pain as a tangible and perceptible phenomenon.
